Ambulatory Geriatrics

Rotation Goals
The goal of the ambulatory inpatient geriatrics rotation is to familiarize the fellow with medical care of older adults in a wide range of outpatient settings, including the geriatrics outpatient clinic, specialty clinic sessions in urinary incontinence, wound care, and geropsychiatry and home visits. Emphasis is on comprehensive geriatrics assessment, health promotion, prevention, and screening for disease, as well as management of chronic care patients in diverse outpatient settings.

Inpatient Geriatrics

Rotation Goals
The goal of the inpatient geriatrics rotation is to acquaint the fellow with the wide range of medical and surgical problems experienced by hospitalized older adults. Working with supervisory geriatricians, the fellow will participate in the full spectrum of inpatient care, including admission, daily notes, discharge, and consultation.

Inpatient care - In coordination with the family medicine hospitalist team, make early pre-rounds on assigned patients on Monday through Friday mornings (no later than 7 AM), round with geriatrics faculty, and manage 4-7 elderly inpatients with supervision. Cover the geriatrics consults during regular working hours, with appropriate backup.

Long Term Care

Rotation Goals
The fellow will evaluate and treat older adults in two long-term care settings, a Medicare Part A and Medicaid-supported nursing home (Peake Healthcare Center) and a private nursing home associated with a continuing care retirement community (Carlyle Place). The fellow will concentrate on comprehensive geriatrics assessment and syndromes such as cognitive impairment, incontinence, polypharmacy, impaired mobility, and depression. The fellow will appreciate the comprehensive, interdisciplinary approach to institutionalized older adults and be able to access appropriate ancillary resources.

Palliative Care

Rotation Goals
The geriatrics fellow will evaluate and treat older patients in a variety of settings, including the office, the home, the hospital, and the nursing home, with an emphasis on inpatient palliative care. Fellows will review the management of common symptoms encountered in end of life care, including pain (physical, psychological, and spiritual), nausea, vomiting, constipation, dyspnea, asthenia, sedation, anorexia, depression/anxiety, and delirium. Fellows will explore ethical and legal decision-making with older patients who suffer from severe chronic disease and terminal illness, advance directives, the Medicare Hospice Benefit, and the impact of Hospice and palliative medicine on healthcare costs.

Rehabilitation

Rotation Goals
This rotation is designed to provide the geriatrics fellow with an opportunity to acquire proficiency in the comprehensive medical care of older adults requiring rehabilitation, in both inpatient and outpatient settings. Fellows will be exposed to physical, occupational, and speech therapy, appreciate the inter-disciplinary approach to optimal care of these patients, and understand how functional improvement is measured and evaluated.
